Graduate Milestone Paperwork

  • Designation of Graduate Committee (.pdf)

  • Qualifying Examination Paperwork- contact the Student and Program Coordinator via email. You should provide the Student and Program Coordinator the date you will begin your written examinations at least two weeks prior to ensure you receive the paperwork required ahead of time. Additionally, when you schedule your oral examinations, you should inform the Student and Program Coordinator of the date at least two weeks prior. To schedule a room for your oral examinations in the LIFE building, contact Deborah.Douglas@unt.edu.
  • Final Defense- you are required to provide the information to the Student and Program Coordinator at least two weeks prior so a formal announcement can be made. These are PUBLIC and require a classroom not a conference room. To schedule your classroom for your final defense, contact Deborah.Douglas@unt.edu.

TA/Scholarship Forms

Travel Forms

  • Biology Graduate Leave Request Form - To be turned in if traveling for a University related event. Also when traveling off campus during a non-holiday break (personal). (Required if you are a TA, RA or GSA)
  • International Travel- It is mandatory for you to register ALL University related, international travel with UNT's Office of Risk Management. Please read each page carefully. Once you have registered, you will receive an email with vital information on what to do in the event of an emergency. Please attach a copy of the Risk Management Email and submit to your Travel Coordinator (Deb, Crystal or Morgan).
  • Receipt Affidavit- Complete this form and submit to Travel Coordinator, if you lose your itemized purchase receipt. This form along with receipts are due to the Travel Coordinator 72 hours after your return.
